Title: Healthy Home, Health Child: The Westside Children’s Asthma Partnership
Description: Healthy Home, Healthy Child is a FREE comprehensive asthma management program for children and families living on the Westside of Chicago. Coordinated by Sinai Children’s Hospital and the Sinai Urban Health Institute, the initiative utilizes a collaborative approach drawing on medical, legal and housing specialists, and incorporating meaningful community input. Once enrolled in the program, a trained Community Health Educator (CHE) will visit the family in their home 6 times over the course of a year. Each visit will include a thorough check of the home for those things that may cause the child’s asthma to flare up and personalized asthma education for the caregivers' and the child. If a child is between 2-14 years of age, lives on the Westside of Chicago and has poorly, uncontrolled asthma, please consider referring him/her to the program.
